Dear Carven:

On Tuesday, January 12, 2010, Integrated Risk Management Associates, LLC did a field inspection at the Muza job-site in Milwaukee, Wisconsin. Our report and recommendations follow:

Gas Light Apartments, 101 N. Milwaukee Street, Milwaukee, WI

Muza employees must install soffit on the overhang of a 6th floor patio. There are no areas where a tie-off anchor can be installed. The roof is finished and rubber sealed. All areas outside the building are finished. There was no way to install anchors inside the building. There is no way to get a boom-lift over the power lines that run alongside the front of the building.

The employees will be working on a flat surface with guardrails installed, but they will be working from the 6 foot level which will put them over the 42 inch guardrail.

There are several technical issues here, but after conferring with Mike Hall, General Manager of Jansen Construction and his safety people, we decided that the safest approach would be to have the employee working on the ladder to tie-off to the guardrail system using a nylon strap. All agreed that this is the only conceivable method and the safest approach for our employees. See photos 1,2,3, and 4.
